2023-24 Sports Physicals
The four RCS Middle Schools are now utilizing a new online sports registration system, Final Forms (FF). If you have not created an account, please use the link above to access athletic registration and upload your MHSAA physical. To create your account click the link https://rochester-mi.finalforms.com/
Students will NOT be able to tryout or participate in any athletic event until their MHSAA physical has been uploaded into Final Forms. Please do not submit any paper copies to the main office.
Please use the link below for the MHSAA physical. The physical must be signed by a doctor on or after April 15, 2023 for it to be valid.
Parents, after you upload your child’s physical, the ADs at the middle schools will enter the examination date the physical was performed and this will remove the message stating the physical is still pending.
Students also need to pay to participate in any sport they are in. This can be found at the following link: Pay to Participate
